COMBINED NOTICE TO CREDITORS AND NOTICE OF HEARING PETITION FOR SUMMARY ADMINISTRATION, FOR ADMISSION OF WILL TO PROBATE, DETERMINATION OF HEIRS, DEVISEES AND LEGATEES, DISTRIBUTION AND DISCHARGE

A Petition for Summary Administration pursuant to 58 O.S. § 246 has been filed in the District Court of McClain County, Oklahoma and you are hereby notified of the following:

1. The name, address and date of death of the decedent are as follows:

Name: Jacqueline H. Eubank, a/k/a Jacqueline Henson Eubank

Address: 8 Farnham Park Dr., Houston, TX 77024-7501

Date of Death: October 23, 2025

2. The name and address of the Petitioner and personal representative is as follows:

3. The probable value of the estate of the decedent as set forth in the Petition is as follows:

Value of the Estate: $325,000

4. Pursuant to the provisions of 58 O.S. §246, the following dates have been set for filing the Final Accounting and Petition for Distribution and for the hearing on the order allowing final accounting, determination of heirs, distribution and discharge:

Final Account Filing Date: Petitioner requests waiver of accountings under 58 O.S. §541.

6. Attached to the Petition is a certified copy of the Last Will and Testament of the Decedent. Petitioner has asked this Court to admit the Will to probate and for other summary proceedings pursuant to 58 O.S. 245, et seq.

8. The Petition sets forth the names and addresses of all known creditors (if any) and heirs of the decedent. Pursuant to 58 O.S. §246, each person receiving this Notice is advised of the following:
9. Pursuant to the Order for Combined Notice entered herein, all creditors having claims against Jacqueline H. Eubank are required to present the same with a description of all security interests and other collateral, if any, held by each creditor with respect to the claim to the Petitioner at the offices of Elias, Books, Brown, & Nelson, P.C., Two Leadership Square, 211 N. Robinson, Suite 1300, Oklahoma City, Oklahoma 73102 addressed to Petitioner’s attorney, Brandon Scott, on or before the following presentment date: the 23rd day of April, 2026, or the same will be forever barred.

Each person receiving this Notice, or any interested party, is hereby advised that you may file objections to the Petition at any time prior to the hearing and send a copy to the above-named Petitioner at the address of his attorney provided below, or you will be deemed to have waived any objections to the petition.

iii. If an objection is filed, the court will determine at the hearing whether the Will attached to the Petition shall be admitted to probate, whether summary proceedings are appropriate and, if so, whether the estate will be distributed and to whom the estate will be distributed.
